Many people with hearing loss tell us the same thing: "I can hear when people talk, but I can't understand what they're saying - especially in noisy places." This common experience highlights why basic hearing tests alone aren't enough when fitting hearing aids. While standard hearing tests measure your ability to detect soft sounds in a quiet environment, they don't capture your ability to understand speech in the real world.
At Applied Hearing Solutions, we include Speech-in-Noise testing as part of our comprehensive hearing evaluation. This critical measurement helps us understand how your hearing works in everyday situations and guides us in programming your hearing aids for optimal performance where it matters most.
Speech-in-Noise (SIN) testing measures your ability to understand speech when competing noise is present. Unlike standard hearing tests that use pure tones in quiet settings, SIN testing evaluates how well you can pick out and understand speech when background noise is present - a much closer representation of real-world listening.
At our practice, we use the Quick-SIN (Quick Speech-in-Noise) test as part of our Best Practice protocol. As noted in our documentation: "Speech-in-Noise Testing is a commonly overlooked Best Practice test. Your ability to understand speech in background noise, with and without hearing treatment, is something that can be measured."
During the Quick-SIN test, you'll listen to sentences at different signal-to-noise ratios (SNR). We present sentences at decreasing levels of clarity - gradually making the speech quieter compared to the background noise. This allows us to determine your "SNR loss" - how much louder speech needs to be compared to background noise for you to understand it.
A person with normal hearing can typically understand speech even when it's slightly quieter than background noise, while someone with hearing loss may need speech to be significantly louder to achieve the same understanding.
As our documentation explains: "The Quick-SIN is a terrific way to identify how much separation of speech is needed above the background noise before you can understand what someone is saying."
One of the most challenging scenarios for people with hearing loss is what audiologists call the "cocktail party problem" - trying to follow a specific conversation when multiple people are talking simultaneously in a noisy environment.
For people with normal hearing, the brain can filter unwanted sounds and focus on a single voice. However, this ability is significantly reduced with hearing loss. Simply amplifying all sounds equally won't solve the problem - in fact, it often makes it worse by amplifying both speech and noise.
This is why patients commonly tell us:
The gap between hearing thresholds (what's measured on an audiogram) and real-world performance explains why some people with relatively mild hearing loss struggle tremendously in noise, while others with more severe loss may do better, depending on how their auditory processing works.
Traditional hearing tests measure your hearing thresholds - the softest sounds you can detect at different frequencies. However, detecting a sound is very different from understanding speech, especially when competing sounds are present.
Even with perfectly fitted hearing aids that restore audibility (the ability to hear sounds), many people still struggle with speech clarity in noise. This occurs because:
This is why SIN testing is so valuable - it gives us insight into these more complex aspects of your hearing function.
Your SIN test results provide us with your Signal-to-Noise Ratio (SNR) loss - a numeric value that tells us how much louder speech needs to be compared to background noise for you to understand it. This critical number guides several aspects of your hearing aid fitting:
Modern hearing aids use directional microphones to focus on sounds coming from in front of you (where most speakers are) while reducing sounds from other directions. Your SNR loss helps us determine:
Digital noise reduction in hearing aids works to suppress background sounds while preserving speech. Your SIN test results help us determine:
Many premium hearing aids have specific speech enhancement algorithms. Based on your SIN test results, we can:
We can create specialized programs in your hearing aids for challenging listening environments based on your SIN test results. For example, if your results show severe difficulty in noise, we might create a program with maximum directional focus and noise reduction for restaurant settings.
Not all hearing aids are created equal when it comes to noise management. Your Speech-in-Noise test results help us recommend the most appropriate technology level:
Oticon Intent uses the world's first user-intent sensors powered by the Sirius platform. This technology captures your listening intentions through conversation activity, head movement, and environmental analysis to dynamically adjust settings.
ReSound Vivia is particularly effective in noisy situations, with its Deep Neural Network (DNN) trained on the equivalent of 25 years of conversation. Its Intelligent Focus feature prioritizes sounds based on the direction you're looking, helping you focus on conversations naturally in challenging environments.
Signia IX provides "advanced sound processing to improve speech clarity, especially in noisy settings" whether in a crowded restaurant or a quiet environment.
Starkey Edge AI uses a revolutionary G2 Neuro Processor with integrated neural processing unit (NPU) that mimics the human brain's central auditory system, classifying complex soundscapes, enhancing speech, and reducing noise all in real time.
Phonak Infinio features dual-chip technology in premium models, including a DEEPSONIC Chip dedicated to real-time AI processing for speech-in-noise separation, offering "up to 10 dB signal-to-noise ratio improvement."
Widex Allure delivers "crystal-clear speech without compromising environmental awareness" with its Speech Enhancer Pro feature that uses a 52-band spectral analysis system to soften unwanted noise while enhancing speech for clear conversations.
Perhaps most importantly, your SIN test results help us set appropriate expectations about what hearing aids can achieve. While modern technology is remarkable, hearing aids work within the limitations of your auditory system.
If your test results show significant difficulty understanding speech in noise, we can have honest conversations about:
Research consistently shows a strong correlation between speech-in-noise performance and overall hearing aid satisfaction. Patients who experience improved speech clarity in noise are significantly more likely to:
By using SIN test results to guide our fittings, we've observed fewer adjustment appointments, reduced returns, and higher overall satisfaction among our patients.
We've seen numerous success stories from patients who previously struggled with other hearing aids but found significant improvement when fitted based on comprehensive testing including SIN assessment. Many report being able to enjoy restaurants, family gatherings, and social events again after proper fitting based on these results.
At Applied Hearing Solutions, we include Speech-in-Noise testing as a standard part of our comprehensive evaluation. We believe this test is so essential that we won't fit hearing aids without it.
Our process integrates SIN testing with other Best Practice procedures:
As our documentation states: "Real Ear Measurement (REM) is a verification method performed by our Doctors of Audiology to ensure that you are meeting your hearing loss prescription for the amount of amplification needed to overcome your hearing loss."
This combination of SIN testing and REM verification ensures your hearing aids are optimized for your specific needs in both quiet and noisy environments.
Understanding how you hear in noise is fundamental to successful hearing aid fitting. Without this information, even the most advanced hearing aids may not be programmed appropriately for your specific needs.
At Applied Hearing Solutions, our comprehensive approach, including Speech-in-Noise testing and Real Ear Measurement, helps ensure you get the maximum benefit from today's advanced hearing technology.
Our audiologists - Dr. Cliff Olson, Dr. Leslie Balderas, and Dr. Kelsey Beck - are experts at using these test results to optimize your hearing experience in all listening situations.
