This privacy policy explains how we, Applied Hearing Solutions, handle your personal information and data, and applies to all of the products, services and websites offered by Applied Hearing Solutions, except where otherwise noted.

Information we collect

Individual settings relating to the Applied Hearing Solutions app are not personal and only stored on your local device.

The app is not user specific, and requires no registration or login. User data is not gathered; unless a user submits a contact request through a form on the Contact page or Request Supplies page. See section "Form Submissions" for information about how your data is handled in the case of a form submission.  In some instances, the device ID might be sent to the backend of the application.

Upon first downloading and opening the application, users will be asked to enable notifications. This is to send general, non-user specific updates from the practice to patients. These notifications include information about practice hours of operation changes, audiologists being out of the office, or occasional general practice updates. You may opt out at any time of receiving such notifications by either not enabling them initially, or by contacting us.

Form submissions

Through the Applied Hearing Solutions app, users may contact our practice by submitting a form on the Contact or Request Supplies page. All information submitted through the forms is securely sent to our practice through Jotform Inc. (Jotform.com: view privacy policy).

When you fill in or complete a form on our application, we collect information relating to your device and the information provided by you in the form itself.

Form responses. We collect and securely store the form responses that you submit via Jotform, who in some cases use third party server providers like Amazon Web Services or Google Cloud.

Please do not include personal identifying information such as your birth date, or personal medical information in any messages you send to us through the app or on our website.

General Privacy Policy

All health and health-related information contained within the Applied Hearing Solutions website and associated Youtube channel is intended to be general in nature and should not be used as a substitute for a visit with a healthcare professional. The advice is intended to offer only a general basis for individuals to discuss their medical condition with their health care provider.
